FPA accepts court orders for competency, sanity, and sex offender evaluations on both adult and juvenile criminal defendants within the area circumscribed on the map above. If you are within our service area you do not need to call before having our office appointed. Just set a due date at least 30 days out and fax the following items to us at 804-739-6725:
A signed court order
Warrants or Indictments
The names and contact information for the judge, Commonwealth's Attorney, and Defense Counsel
Please indicate how to reach the defendant (name of jail, or last known telephone number and address)
For competency and sanity evaluations on adult defendants, request VA Supreme Court form DC-342; for juvenile competency evaluations, use form DC-522.
A model order for a sex offender evaluation will soon be posted here.
Please send us as much other background information as is possible. Per statute, that includes: (1) the reason the evaluation was requested, (2) offense reports, (3) a summary of the defendant's criminal history, and (4) mental health records to the extent they are available.
FPA's doctors travel to the jails, detention centers, prisons, and group homes within the area outlined on the map above. If you are not sure if we will come to your local jail, see our list of jails to learn more about where go.
For defendants who are not incarcerated, if they are mobile then we prefer to meet them at our outpatient office in Chesterfield County. However, many indigent defendants have a limited ability to travel. When that is the case, our psychologists can meet with the defendant at an appropriate place such as the defense attorney's office or a conference room in the local courthouse.
Except when there is a special request, the cases are assigned to the psychologist who can get it done most quickly and who is also the best match for any issues which are specific to the case.
The map above is only for court-appointed competency, sanity, and sex offender evaluations. For privately retained evaluations, sexually violent predator commitment hearings, and capital sentencing assessments, we can travel further. Please call 804-739-4669 to check on our availability.
You are strongly urged to call first before issuing any subpoenas to ensure there are no conflicts for the date of your hearing.
